What is the review process for applications?Applicants will submit an application online at www.matchmadeintherapy.org/hopefund. Match Made in Therapy will review the information submitted to determine if any additional information is required and verify the information. If the applicant is determined eligible for debt relief, the payment will be calculated. Once completed, the applicant will receive notification of eligibility, and payments will be made directly to the business, hospital, treatment center, and/or health care provider.

How does Match Made in Therapy determine eligibility?To be eligible you must: Have outstanding behavioral/ mental health medical debt. This could be debt from hospitalizations, doctors' offices, treatment centers, or rehabilitation facilities. Earn an income at or below 50% of the area’s median income (AMI). Have physical documentation of bills with legal names and expenses listed. Provide proof of insurance policy or explanation of no insurance. Once an applicant submits all required documentation to respond to the requirements, Match Made in Therapy will review that documentation to confirm applicant eligibility.

How will my assistance be calculated?Determination of assistance amounts must be determined by a physical or electronic bill, signed by/ given from an established business or organization that identifies the applicant received services. Match Made in Therapy will reach out to the organization to verify that the payment is still outstanding. Proper documentation : Contact information from the treatment facility. Electronic or scanned physical document of the bill. Collections receipt from the bank. If an applicant does not have the proper documentation they may not be eligible for assistance. Please provide as much information as possible in your submission form. Email all bills and documentation to info@matchmadeintherapy.org with the subject line “Hope Fund Documentation- (FIRST NAME LAST NAME)”

When will I receive the assistance once approved?Assistance is provided through our donations and the funds raised by Match Made in Therapy events. Assistance will be given on a first come first serve basis with respect to the number of funds requested. Match Made in Therapy will disburse payments accordingly.
